(Intensive Activity Assessment or Respite Support for under 16's and over)

Open choice is now in a position to offer a fully inclusive activity break from weekends upto a maximum of two weeks. This is an ideal opportunity for young people/adults requiring a platform between services or providers.

Open choice can also provide various professional assessments during this time depending on needs; our completed assessments would be handed to the appropriate social worker once the activity break has ended.

During this activity time Open Choice will provide two of its experienced staff members who have current and up to date CRB's with a 3rd staff member on full standby. Open Choice's staff will also be in daily contact with management.

Open Choice's Risk Assessment Form will be required (please see Referral Procedure link). Also, please list any criminal offences / convictions, as well as details of any mental health issues or learning difficulties.

Lake District or New Forest

Open Choice is now in a position to offer weekends upto a maximum of two weeks availability at either of the above destinations / areas in a comfortable country cottage.

This opportunity is ideal for the purpose of respite or assessment depending upon requirements with a two to one staffing ratio.

Open Choice is providing this service as a transition period between services and providers, alternatively a break out of county.

Open Choice will also provide a 3rd staff member on standby for the duration of the trip, plus management on standby 24hrs a day.

All Open Choice's staff are suitably experienced with current and up to date CRB checks.

Open Choice's Risk Assessment Form will be required (please see Referral Procedure link). Also, please list any criminal offences / convictions, as well as details of any mental health issues or learning difficulties.

Narrow Boat or Barge

Open Choice is now in a position to offer a Narrow Boat / Barge a weekend or up to a maximum of one week, for the purpose of respite or assessment with a 2 to 1 staffing ratio.

This opportunity is ideal for transition periods between services and providers, or simply a break travelling our local water-ways.

Assessments would vary depending upon requirements, e.g. a young persons/adults idea of and level of semi-independence would be explored for possible further services from Open Choice.

Open Choice will also provide a 3rd staff member on standby for the duration of the trip, plus management on standby 24hrs a day.

A round-trip begins at Leighton Buzzard, Bedfordshire.

Open Choice's Risk Assessment Form would be required (See our Referral Procedure link ) and please state whether the young person/adult is a swimmer or not. Also details of criminal offences / convictions would be required, including any mental health issues or learning difficulties.
