Referral ProcedureFor Independent Outreach Services Only - our Risk Assessment Form is required. Before Open Choice can consider a Referral the following information is required: • The completion of Open Choice's Referral Form 1 (See below) • A current and up-to-date Risk Assessment (Open Choice's is available - see below) • Criminal Charges, copy's of all Records • Full previous placement History Mental Health Information:Assessments current and up-to-date, Hospital Reports (diagnosis) Care Plan, Follow-on treatment, Medication, Psychology Reports, What Mental Health Team is now involved with young person / adult, if any (How often will they visit the young person / adult) and What work will they be doing? Open Choice ask for ALL the above Information to be compiled and forwarded by post or email at the one time. Once all referral information is received a member of Open Choice's Management Team would like the opportunity to meet with the young person / adult. A decision as to whether the young person / adult would be suitable for Open Choice will be reached within one week of receiving ALL Referral Information listed above and arranging the necessary meeting. A figure of fees will also be given at this time and will NOT increase or decrease throughout the time scale required.
